Can you give a little more information, like what does the pbx log say. In the pbx logs in admin mode, turn off all other logs except TFTP log, which you should set to 9. Then just do the provisioning by resetting the device to factory default, then directing it to the pbx (either manually or through option 66 of the DHCP) and give us the logs.
