Outbound Failed Calls

The following call captured in the log file failed twice in a row. This is the SnomOne CS410 appliance. The Carrier is Comcast with a 4 port DOCSIS IP to FXO gateway from ARRIS. 200 good calls are made in a day, but the clients only remember the ones that failed.

Can you shed light on what might cause the Unobtainable Tone Detection Errors?
